a) A photocopy of the requestor's USJA Coaching Education Certificate of Completion

b) A statement of your name exactly as you want it to appear on your Coaching Badge

c) A passport-sized photograph of the requestor

d) Address to which the Coaching Badge is to be sent upon completion

3) Send USJA Coaching Badge requests to:

Alternatively;

The requestor may fax the USJA Coaching Education Certificate of Completion, name and return address information to 719-633-4041. On the fax cover sheet;

1) Send your fax 'To: USJA, Request for USJA Coaching Badge'

2) In the 'From' area of your fax cover sheet include;

a) A statement of your name exactly as you want it to appear on your Coaching Badge

b) Address to which the Coaching Badge is to be sent upon completion

Do Not fax photographs! Digital photographs of the requestor may be emailed to usja@usja-judo.org.   When emailing digital photographs;

1) In the subject line, state 'Request for USJA Coaching Badge'

2) In the body of the message text include;

a) A statement of your name exactly as you want it to appear on your Coaching Badge

b) Address to which the Coaching Badge is to be sent upon completion

Please allow 4 to 6 weeks processing of USJA Coaching Badges.
